Native and Indigenous Community Survey
This survey is intended for Alaska Native (AI/AN) and Indigenous people and their families who live in the Tri-State area. A history of behavioral health services is not required to participate. Additionally, a Tribal card is not required to participate.

We are interested in gaining a better understanding of the behavioral and mental health services that are
available for AI/AN and Indigenous people in the so-called Tri-State area, as well as their priorities for services.
Participation will take about 10 minutes. The survey is ANONYMOUS, which means that no one will be able to link your name with your answers so your responses will be private and will not be shared. There are no right or wrong answers. We are grateful for your time and participation today.

If you have questions about this survey, please contact our Co-Director by email at ariel@urbanindigenouscollective.org.
